Amex A01 is triggered when the amount settled to the cardholder’s account exceeds the amount that was originally authorized. Every Amex transaction must be settled for an amount that matches (or is within tolerance of) the authorized amount. If the settled amount is higher than the authorization, A01 follows.

The most common cause in card-present environments is tips added after authorization — a restaurant charges $80 at the table, the customer adds a $20 tip on the paper receipt, and the settlement of $100 exceeds the $80 authorization. Amex allows a tip tolerance (typically 20% above the original authorization for food service), but exceeding this tolerance triggers A01. For e-commerce, A01 is usually a system error — the settlement amount was modified after authorization was obtained.
